My observations tend to be more catch as you can sorts of things...

As the child does something and I scribble it down and write it up later.

There are some pro formas for obs sheets on the Eyfs CD if you have a nosey around :D

I like the ones on the EYFS disc. If you go into the green section Enabling Enviroments,then on the right hand side theres some bits you can highlight, I usually use the 2 year old planning and (I think) the 3-4 year old planning bits...theres lots of sheets you can try and use whatever suits you best.
