Hi everyone,

I have filled in my registration forms as well as i can but have a few questions.

1. On the health declaration form it asks about medical problems in the last five years. What do they want to know about, is it everything I have been to the dr about because i honestly dont know if i can remember it all, also i have been to the hospital about a minor gynaecological problem which resolved itself, do i need to tell ofsted about that?

2. Who can i use as references? I only really know family, there's possibly one friend i could ask for a reference but she's not hugely reliable so it wouldnt get sent off quickly if at all, and everyone else i can think of are family or passing acquaintances. What about my step mothers daughter we arent blood relatives but have always thought of ourselves as sisters.

I don't want to send off the forms with incomplete information.

Any advice will be massively appreciated.

They will write to your doc so... if the doc will mention it then you should too or it might look like you are hiding something

I don't see why not! It's anyone that can say you're not a mad axe murderer really and reliable with children

Thanks Sarah. That was what I thought, although nothing could have an impact on the care i provide i'm going to write it all down anyway. Means a trip to the dr to look at my own records though lol. And I think i'll ring ofsted and ask about the referee problem, I have one sorted out it's just the second one that i'll have problems with. If they say anything worth knowing i'll post it on the forum, hopefully it'll help someoone else then too.
