I use the Every Child Matters agenda when I'm asking parents questions...
Do you feel I am providing your child with healthy and nutritious food, drinks and snacks?
Are you happy that I contribute towards keeping your child healthy?
Do you feel your child is safe when s/he is with me?
Do you think the activities I do with your child help them to enjoy their time with me and achieve to the best of their ability?
We do a lot of multicultural activities and visit places in the local community to help your child make a positive contribution to the world around them. Are you happy with this aspect of my care?
Do you feel I listen to your views and work closely with you?
Etc x:D

Spot on Sarah - thats the sort of thing I have in my files.
I also ask them things like:
Have you seen all the relevant documentation - such as First Aid Cert, OFSTED registration, Public Liability.
Have you received a copy of the policies and procedures for the setting?

Hi
Ask the parents to write them like a thankyou letter for all the activities, outings, happiness etc.. that you do for the children.
This is what my parents did for my nvq3.
