Do you complete a written risk assessment every day or a visual one?

Everyday as this (I believe) has to be dated. I just have a tick sheet (well I initial it) with a list of things to check. Its one sheet a week.

Yes I have an everyday check list for every room I use and I tick it off before I start work in the morning.

Do anythings ever NOT get ticked off? And do you have to then write what you did about it? How long do we keep these for?

I have an actions needed/actions taken column at the end. I would use this if something can't be ticked off to say what I did to make it safe for that day and if there is anything more that needs to be none, not that I've used the column yet.
