in an attempt to earn a little extra over half term i'm offering 3 afternoon play sessions for children aged 4-10 at a discounted rate.

i haven't had much interest yet but i was thinking ahead and obviously i'll have to get certain permission forms signed and a child info form filled out. i usually use ncma child record forms but i could potentially have 15 new children during half term for 3.5 hours each. would it be ok to write my own forms for these children? i would use exactly the same questions as the ncma ones.

I write my own for ocassional care so can't see why not. I've done something similar in that I have specifically put an advert in the postoffice as a childminder offering holiday only cover, odd days, paid by the hour. Just to try and build up the business.
