Hi sent in my CM1 and CM2 form in with the medical form, signed by my GP.
Was rung by Ofsted and asked a few more questions that were part of the new forms.
BUT, why have they sent me a new medical form? It is exactly the same as the original and it's even worse as its a very bad photocopy.
Does this mean i have to get the GP to fill it in AGAIN and pay AGAIN?

Why dont you give them a call and ask if they received your first copy?
