It is a Self Evaluation Form.

Every childminder is being asked to complete one by Ofsted, it is not compulsory BUT if you don't do one then you will have a far longer inspection than if you do.

When you are registered you will be sent details of how to access it on line or how to get a paper version.

And you are not dumb! You probably haven't had much info because you are pre-reg :)
