I have a mileage log book from staples. It has
Driver
Date
Journey from/to
Start mileage
End mileage
Business/personal
Its amazing how those little journeys add up

I have a form that I have made myself.
In my diary I keep a list of where I have been and on the form I enter the total mileage for the day. From here the form automatically works everything out. It is dead easy but it does work for me which is depreciation.
If you are just claiming mileage at .40p per mile, just keep details as date, where you have been and mileage total. You always round upwards.
