I am filling up NCMA contract right now and I have silly question regarding page 2 charges... If I am charging per hour shell I fill up daily rate weekly etc or just hour rate and thats it... ????

I only charge an hourly rate so I tend to put N/A in the other boxes. Don't know if that's right or not though.

Fill in every box and then you are covered.

I put an hourly rate, daily rate and monthly rate in and they will all be different. it is between you and the parents which way it is worked out.

I always get paid in advance on the first of each month for a month

I put an hourly rate and n/a for the others and I get paid one month in advance on the 1st of the month.
