I would like to give my ex-mindee's parent a questionnaire evaluating my services so that I have something to show perspective parents. does anyone have an example or know where i can get one? i was thinking about spliting it into the ecm principles so it would link directly to my sef?

Are you happy with the way I keep your child healthy? - think about the meals I supply, the way I record medication etc...

Are you happy with the way I keep your child safe? - think about the risk assessments I share with you, the way I record accidents etc...

Something like that following ECM perhaps and giving them prompts so they know what to write about? That's the way I do mine :D
