I have mine in a word document with two columns. I then drag a photo onto the page into the left hand column. I write the observation next to it, in the right hand column.

I do similar but have 3 columns in a word document - left hand for photo, middle for observation and the right has all the development matters (Or EYO) headings and characteristics of learning which I just highlight. Bottom of page is space for next steps and ideas for parents. I complete mine on the computer, insert photo, print and highlight the DM headings.
