Does anyone have a form that they fill out for children who suffer from allergies - i.e. what they can have to eat/drink, what they can't, symptoms, procedure to follow in relation to medicine, etc????

I looked after a child with complicated allergies. I got the parent to write a letter with all the details of how to manage the child's condition. I read it through with them and added notes to it if needed and we both signed it. The letter was them stuck up inside a kitchen cupboard (with the name covered by a post it) so I could refer to it quickly if I needed to.
