I am going to fill my form out sooner than later was going to wait till end of July however decided otaku plunge and aim to sending off next week. Do I need to send off my health declaration form with it. The crb or DBS that's needs to b done is that correct, I'm terrible at filling forms out. I'm dyslexic. Amount of times I've had forms sent back to me as I've missed them out. :-/

I was going to do the same as I'm currently doing my cm reg course, however it states that your application isn't complete until you have sent them all your forms plus copies of your certificates. So I think it's a waste of time at min until you at least have certificates back, I don't want to think that if I send it off now that it could become lost at the bottom of someone's desk!

My doctor took weeks to do my health dec so I sent the other forms off and the health one later

Do I add my first or last day of attending first aid as I don't have that back yet and form asks for date gained?
The cr2 forms, am I correct or completely wrong in thinking that these are my crb forms?

I can't remember exactly what the form says, but i was sent dbs forms after I had sent the ofsted form and paid the fee

I would put the last date as that would be the date you finished the course, I wouldn't worry too much if the date isn't exact as ofsted check your cert when they do the pre reg
