One of my mindees is starting school in September. He currently comes to me for 10 hours a week and attends preschool 12 hours a week. Do I need to do a transition form for school as well as preschool or does preschool do one and I contribute to it?

I would have thought pre school would take on this responsibility due to mindee spending the longest time their. However I would speak to pre school and see what they say. It may be easier if you both do it together.

Children can often do different things / have different experiences wherever they go so I would do something quickly myself :D

I have mindie here more than pre school, but were going to work together.
If they won't work together I'd suggest doing one aswell.

Thank you everyone. Preschool haven't been particularly good at working with me for the past 2 years! I will speak to them about it but if they don't want to work together, I will write my own.
