Do you complete transfer notes for when you have a minded child leaving??

I have a 2 yr old leaving as parents are moving away, .... would like to do tranfer details for their next setting but don't really know what to include.

Does anyone have any forms for this??

I would just write a little letter with some info about how fab the child is and how well they have progressed while with you.
