Hi everyone,yesterday I have filled in my application online to be a registered childminder but was wondering about the Health Declaration ? Do they send me this or do I download it from the Ofsted website? Don't want any holdups !

I got mine as a paper copy with my Ofsted application (also a paper copy given out at the course). Here is the PDF version on their website. Ofsted | Childcare registration form - Health declaration booklet (http://www.ofsted.gov.uk/resources/childcare-registration-form-health-declaration-booklet)

I assume you print out, fill out and give to your GP with a pre paid envelope. They will send directly to Ofsted unless you request to see what gp has written. I got a pre paid envelope with my paper copy so you may have to supply your own.

Thanks for that.Have rang Ofsted this morning and they send a paper copy out.i don't know why I can't just download one now from their website and take it to the docs.They said the downloadable one is in case the original one gets lost! :panic:

How strange, oh well it will save you printing it! There's comes as a booklet so may be better off using that anyway! Plus they provide a pre-paid envelope! :D

Dont mean to sound negative but if you get the option double check what the doc writes. Mine filled it in for someone entirely different :-)

Hand your form in to the Dr but dont give a pre paid envelope, just say you will collect it. That way you can check what they have written is correct, plus you know its been posted and not filed somewhere :)

I downloaded the form from there website and got my dr to fill that one in and send it off, hope I haven't done it wrong now and they still accept it

Where do you send it off to? Just the Ofsted address in the form? If so, what do you put on the envelope? I downloaded mine and took to the Dr who gave it back to me. I'm clueless about it and because he didn't charge me, I didn't question it. Now not sure whether to put my information on the top. I don't want to send it off and it gets lost.

Sorry about all the questions!

I gave mine in to my gp yesterday and was told it'd be £65 for them to do it! So if you've not been charged for yours then that's brill. 😊

Well I picked up my health booklet from gp and was told I won't be charged for it after all! :)

Think yourself lucky... My Gp wants £150 and it's taking for ever for him to do it, so far I've been waiting 4 days!! You would think they would be quicker at that rate of income!
Once I have it, can I then register to ofsted? X
