I can't remember exactly what's on mine but I've covered:
If parents are happy with the way we communicate, the way I manage behaviour, if they're happy with the routines, meals, activities provided or if they would like anything done differently. Also to comment on how they think their child feels about coming to the setting. Can't think what else is on there but hope that helps you a bit

Hi,
I had an ofsted inspection at the beginning of January, the inspector liked my parent questionnaires but suggested I change the question 'Is there anything you would like me to do with your child?' to 'are there any ways you think I could improve?' The emphasis seemed to be on 'improve'.
I also had some child friendly questionnaires which consisted of are you happy here: all the time/most of the time/sometimes/not at all along with picture faces, what do you like doing here? Is there anything you don't like, What would you like to do? I am lucky as both sets of parents helped their child to fill in the questionnaire and the inspector really seemed to like this.
The inspector was also looking for my response to any of the comments so if you do some sort of evaluation of the comments it shows you are evaluating your service.
Hope this helps :)
