Hi, this is my first post and really need some advice please?

I've recently completed the CYPOP5 online and have an appointment with the CSSIW to take my application form in this week. I have only just come across the questionnaire to assess the suitability of the registered person and i am completed confused.

A lot of the questionnaire seems to be for day care settings, do i just ignore the questions that apply to the day care settings?

Am i expected to hand this form in along with the application form? This wasn't mentioned when i called and spoke to the CSSIW.

I am going ring them again in the morning but was hoping someone may know the answer, the whole process is started to drive me mad.
