Just following up from the thread by perky ears about the parent questionaires how do you all do these? I am just in the middle of doing my first ones, do you have actual questions written down, if so what sorts of questions? or do you just ask for a general little write up? im unsure how to proceed with this. I dont want to bombard parents with yet more stuff to do but would appriecaite a litttle feed back to store on file?

i have a few main areas for each question, I follow up by asking, either 'please expalin why?' or 'do you have anything else you would like me to do?' so they can't just say yes or no ;)
so i.e.
how do you feel I :
help children to be independent
to keep and teach them to be safe
provide and teach them about healthy eating
are you happy with:
range of resources and activities I provide
the level of info I provide
do you feel my service is:
professional but friendly
value for money
overall what do you like about my setting
what does your child like
any other comments about my service
