childminding network

Hi, I contacted my local schools to inform them to go on the list of registered childminders, and was told I needed to hand them in a form from the childminding network???
Could anybody tell me what form this is and how I can get it?

Didn't want to read and run but could you be a bit clearer ... sorry it's prob me I've had a very busy day and am just chilling out.

I'm part of a network as I'm an accredited cm but I don't know about the school thing :(

When I signed up to the childminding network (Bristol) I got a certificate to say Tara xxxxx is a member of the Bristol childminding network. I recied another when I became enhanced provision. Maybe they want a copy of that x

In our area we all recieved a form from th local DO:thumbsup:

I'd ask the school again exactly what form they want
