Outting risk assessments

Morning all,
I'm on the scrounge lol! My brain is frazzled with paperwork at the moment and I was wondering if any of you had a risk assessment template you use on all your outtings? Its the only one I havn't got in place and really need to find something that perhaps I could take with on each outting.
