Has anyone recently become accredited please? I need help with filling out the Assessment form, I don't no how much information to put on the form or is the CDO looking at the evidence? Can anyone help me please? :

what was the question ....... if you fill it in you DO will fill in any gaps with you

Every area has its own forms so the questions you have will be different to the ones I answered for example.
However I am sure our members will try and help if you are stuck! :D
