I am just going through Section 2 for the registration form for the CSSIW.
What information needs to be on the Child Record form?

Everything about the child including age, date of birth, preferred name, family details, health, other provisions attended, religious and cultural requirements, family wishes, home routine, previous learning experiences...

Do a Google search there are lots of examples out there :D
