Accident book

Hi all, I'm trying to find a new accident book, I want one that uses carbonated paper so I can give parents a copy but also like the morton Michel ones that have the body silloette so I can mark on where injury was. Any advice please.

If you like the MM one maybe that is the one for you.

I had to order a refill for NCMA one and they have changed the layout - it's horrible!
No more body shape and 2 accident forms per page but only space to put one name :angry:

They have the body form on a separate sheet sheet now. Section 5 :thumbsup:

They have used this format for at least a couple of years.
