Updating policies! :( at the moment i ask for name and address on each policy...do i need the address on each policy? Already have full details of child on initial record form surely i don't need it on the rest???

I don't have a name and address on any of my policies. I have formed mine into an 8 page document with a signing sheet.
The signing sheet has all my permissions on incl medical and first aid, a list of all my policies with the statement ' I have received a copy of, have read and agree to'.
That they have received a copy of my Ofsted report and insurance details.
The parents then sign, print their name and date the one form, no address needed though.

I have an 'I have received, read and understood the P & Ps' statement with my permissions form and parents receive a copy when they have signed up.
I don't put their details on the P & Ps - they are mine.
