I have just signed up to use a system where I can store childrens details and learning information (a bit like Noble Minder)
I need to write a premission form for this and have no clue on how to word it.

Has anyone else done one that they dont mind letting me borrow for inspriation?

The online system I use has one incorporated on their list of permissions for parents to sign

Which systems do you ladies use?
