First time given notice, do I do letter as well as verbally

I would just write it up very simply and post or hand it to her in person. Keep a copy.

Something like "following on from our conversation earlier today I am giving you 4 weeks notice as of x/x/x (todays date). The final date of care will be x. Final payment of £x is due on x/x/x.
