View Full Version : What information to get from new parents?

Hi there, I'm just registered and needing some help! I have 2 children starting with me next week and will need to fill in all paperwork etc on monday, my cco (or whatever they are now!) gave me a lot of examples of forms but not any 'All about me' forms or parent/emergency contact forms. Does anyone have any pointers? Don't want to go overboard with information but at the same time need to make sure I have enough.

Hi Fiona,

I am newly registered and got SCMA record binder which saves me from drafting up most forms (this year at least).

If you mean the information to have on child (as SCMA also has an all about me form which is generally for when they have started care):

Name
Date of birth
Parent/guardian name and address with full contact numbers etc.
Place of work (parent)
tele numbers.
Place of work.
Emergency contact details, who will collect child.
Doctor and health visitor details.
Address of doctor etc.
Special dietary requirements.
Childhood illnesses already had.
Allergies/intolerances.
Immunisations/vaccinations.

Then it is recommended that you take details of child's individual needs via a form as well i.e. likes/dislikes, food likes and dislikes, toilet capabilities, dressing capabilities.Sleep pattern if applicable. Normal routine and whether parent wants this to be strictly adhered to or is flexible.

Its a fun way of gettin info. Then use a NCMA child info form to get the important bits like doctor, immunisations, emergancey care permission xx

Thank you ladies, that was just what I was looking for, first mum has just filled everything in and signed contract, can't wait to start now!
