Yes you have to get parents to sign to confirm who has it. If you use NCMA Contracts there is a space on page one for it.

You need confirmation in writing of who has parental responsibility and who has the right to legal contact - there's no set place where it has to be recorded.

There is a form in free resources (free downloads) if you want to record it separately or don't have NCMA contracts.
