What does she want one for? an incident form is what you would fill in if anything happened to a child, it is similar to an accident form, the NCMA accident, incident and medication folder use the same form for an accident or incident. You record child details, record of what happened condition of child after eg upset, quiet etc, any witnesses and both you and parent signs it and parent keeps a copy.

has something happened?

an incident report form is something YOU fill in -not the parent.

its as QualityCare describes

no she asked for an incident form

as the others said ,you fill in the incident form and she signs to say she has been informed ..did you tell her about the `incident` or was she present when it happened ?
