Pre-existing injury form

Hi,

Does anyone know what I'd need to include in a pre-existing injury form please? I know you can get these from the NCMA but does anyone do their own?

I make my own up, it has on it

- child's name and dob
- date, time and place the injury occurred
- description of injury
- parent/carer's description of how injury happened
- if appropriate the child's version of how the injury happened
- record of any treatment already given (what, when and by whom)
- record of any further treatment required

Signed and dated by me, parent/carer/child (as appropriate)
