I have this form I use for when I am doing holiday cover or a temp or short-term contract. I think I based in loosely on the MM child record form!
Would you say it includes all the info I need or would you add more things? I have been using it mainy for holiday cover when I only tend to have the children for a few days. For long term contracts I usually have a child record form, a list of permissions and a contract. I'm wondering if this would be sufficient to cover child record & permissions on one form, so I only need to do a contract separately.
Any comments (good or bad!) would be appreciated.
