permission forms

i was thinking of doing one generic permission form for each child with a yes & no box for parent to tick which they are happy with.

what permissions do we need to obtain? does anyone have a one i can look at?

I have lots of ideas for permissions (a lot more than you will ever need) on my www.eyfsforchildminders.webnode.com website.

The essentials are ones like outings, photos etc.
