How often MUST we review these if nothings changed in the meantime? and for the ones like library visits and soft play do we have to do a new one each time we go?

As far as I know there is not an actual timeframe in the guidance to say how often they should be reviewed. I would say every 6-12months or before if necessary.

No you don't have to do a new one for each time you visit but there might be things to ammend or add on your original ones.

That's my understanding anyway. :)

I review as neccessary , things change with seasons , new children etc. I date and initial each time I review even if nothing has been amended . Each half term works for me. The main home / garden RA has to be at least annually , or again if there are any changes.

The Eyfs says to check your outings RA every time you go on an outing.

Update if something changes :D

I write on my RA if I notice anything on the way. Like on preschool run, there was building works going on in a garden. So I noted that and said to use other side of road to avoid flying debris hitting children. Then noted when it had finished.

I check house every 6 months but again note meanwhile if anything happens
