risk assessments

HI all

I'm just going through the process to become a childminder and have my visit from my childminder coordinator on thurs to check over all my paperwork. The problem is my risk assessments I don't like the template they have given us to use and was wondering how everyone else sets theirs out. Any advice would be gratefully recieved thanks.
