Hi I don't like the MM medication forms so am doing my own medication book.

What are the essentials?

Name of child
Date
Medication
Amount
Last given
Parent sig before and after administered
My sig after administered

Anything I'm missing??? Thanks

Storage instructions... shows if it needs to go in the fridge etc.

Use by date... shows you have checked it in case of old bottles being sent.
