Hi!

I'm new and just trying to create the required permission forms, however I was wondering if a child comes from a two parent family, do both parents have to give permission? And if it's a one parent family is it only the parent with care? I only ask as on the ncma website the forms have two boxes for parents to sign:confused:

Would be very grateful if anyone could clear this up for me! Thanx in advance

Its always an ideal to get both parents where possible to sign. That way you are double covered in all eventualities. (for instance both parents have signed the contract therefore BOTH parents are liable for the fees)
Its not mandatory though.

i get both parents to sign contracts and parental responsibilty but either of them to sign everything else. x
