Contract help PLEASE!

Hi, I am a new CM and have been asked to provide 'emergency' care for a baby from tomorrow! (for a few weeks) I do not yet have my tool kits from the insurance company and so I have no contract for the parent to sign. I have permission forms for her to fill in. Can anyone please direct me to a place I can purchase/downlaod these forms? I am happy to pay.

Is there a childminder local to you who could loan you a contract until your toolkit arrives or maybe your development worker could help you out. If not, where are you located maybe a forum member could help you or, if there is time, if you pm me your address I will post you one tomorrow.
