Is this ok for individual planning? Do I need to specify what areas I will be doing obs on?
Any ideas for improvements would be appreciated

I depends on how often you see the child and what the child is doing / saying!
If they are part time I would say 2 or 3 thoughts for individual planning a month...
Plus of course if they join in with group planning then you'll individualise that for them.
You do it automatically, you give A playdough rather than paint because that's what he prefers etc.
If they are full time then you'd spot more happening and probably do a few every 2 weeks or so.
But even then some children don't seem to do much developing for a while then suddenly everything happens at once.
So you would potter on with them and then have a rush of observations and assessments and next steps to write up.
If you are feeding in what parents are saying and you have a good, chatty relationship with them, then a lot of your job is done for you!
Hth :D
