planning sheet please?

Does anyone have a long term planning sheet I could use. I haven't been able to figure out how to adapt a spread sheet for what I want.

I would like it landscape, months down left hand side and week 1-4 across the top with little boxes for me to write in. If anyone has one I would be really grateful.

oops just re read your post .....you can change the headings in the rows to suit what you want:)
