Does anyone have a risk assessment for dogs they dont mind sharing

Am just trying to get organised and write up my RA for the new puppy before the kids come back on Tuesday. Sure Ive missed lots though, being new to this dog owning malarky :blush: . Does anyone have one they don't mind sharing? Or ideas what I should be putting in there so I can see what Ive missed.

Would me really grateful for any help :)
