need help wording a letter to parents, please help xxx

i need to write a letter and get them to sign to allow me to leave my hubby in charge if i out, school run or something, can you help word it please, not sure how to start..

Dear .......

I have registered my hubby as my assistant and can e left in sole charge of the children within my care, for such reasons like school runs, doctors appointments , emergency etc.......

I ............give permission for the assitant, Andrew, to be left in sole charge of my child, i understand that he is a first aider and fully qualified.
