If the parents are married and living together I only get one of the parents to sign it as usually only one of the parents comes to see me as the other is working.

I try to get both parents to sign but sometimes for various reasons it's not possible.
The most important signatory is the one who will pay the bill!! :D

On my permission forms I ask for the names of both parents and then I say that signatory number 1 will be the main signatory for the permissions and at the end of the form I get signatory 2 to confirm that they have also read and ageed with the permissions.
That way one poor parent gets to do a load of signing and the other only has to sign once ;)
