Children's evaluation form

Does anyone have one they would mind sharing? I would like to know what the children think about my care, setting, what could be improved what they are looking forward to etc.

I ask the children these questions on a form, they can fill them in while they are with me or take them home to fill in with parents help. I also have an 'Idea Tree' which I encourage children to write or draw on post-it notes to stick on the tree, the things that they would like to do.
What do you enjoy doing most here and why
Do you think you can choose what you want to play with here
Do you think that you help me to decide what activities we do
If you want to change the toys or activities you are playing with, can you do it easily here
Is there anything else you would like to do but cant do here
What things do you like to do outside here
Do you think you play outside enough here
Are there things that you would like to do here, that you dont already do here
If you want a drink, do you know where to get one, and can you help yourself
Do you like the food here
Which are the best things to eat here
Do you feel safe here
How do you know when you have done something well here, who tells you
Is there anything that you dont like here
Is there anything that makes you feel sad here
Who can you talk to if you feel sad or unhappy here
Can you move around here when you want to
What is the best thing about coming here
What is the worst thing about coming here
