parents questionaire

Hi not sure if this is the right place to ask but has anyone got a parents questionaire that they hand out and if so could i have a copy of it please. I have just been to a childminders support group and for our next session we need to take one with us. As i have only just started minding i haven't got one yet and im not sure what to put in it :blush: so was hoping one of you wonderful ladies could help me.
