Been seeing this on here and was wondering what sort of things you put on them? things like how am i doing? can i improve etc..what else though?

PMSL ohhhh sorry yes make a list of what you would like to know,,, and ask rather than send out questionnares as i never get mine back lol

These are the ones I used for my parents and children (under 5's and over 5's). Also a friends one written for her Foundation Degree with the EYFS in mind
