Hi all

Could anyone please share a permissions form with me? Its been a while since I have updated mine and I would like to make sure I am on the right track!

Hi. I use this one, I changed it a little but it was from the NCMA website.

Sarah these are fab and your website is brilliant! Is that how you set your permissions out so could I just do them exactly as they are on there?

I set them out in a form... with a yes / no box for each one on the right hand side and a signature bit at the bottom :D

Oh gosh ok thanks Sarah - may I ask how you do a yes no box?

Sorry I didn't explain that very well... :rolleyes:

2 columns...

1 Permission for...

breathing

sleeping

wild parties

playing

etc

2 Yes / no - a very thin column just enough room for parent to write yes / no

And an instruction bit - please read the permission and state yes or no in the right hand column. If you do not agree or you are not sure, please discuss with us before completing. Some of the permissions are statutory requirements of the Eyfs ... something like that :D
