Can i write one which the child answers?

Obviously with Parents signature at the bottom?

I have an 8 yr old starting and I thought it might be nice for her to write abit about her self?

cannot see why not but you need doctors details etc, why not make a form for the childrens interest and food likes , hobbies, books and games, so they can be envolved

I have a seperate form for the school age children, asks then their likes, dislikes, what they prefer to be called etc.

I send forms home so parents and over 8's can fill them in together.
