I haven't got a review date on mine as the reviews are ongoing. I do have a date printed and a version number which changes each time I change the policy

I just put a review date on my signing sheet. I don't have my policies printed on seperate sheet, they re printed out to form a complete 7 page document with the Permissions and Consent sheet at the front and the Complaints procedure as the last thing.

The Eyfs says that -

The provider must conduct a risk assessment and review it regularly – at least once a year or more frequently where the need arises.

It is considered good practice to put a review date on everything - policies and procedures, risk assessments, paperwork etc... then diary it to show when you intend to do it.

Hth :D
