Bouncy castle Policy and Permission form

Hi

Does anybody know at all if I need a Policy and Permission form for my mindee 12 months to go on a bouncy castle. I think I need both and a risk assessment, just wanted to check if i was on the right track. I would be grateful if somebody could confirm this
